Obesity is More Complex Than We Think It Is

I've previously criticized the nutritionist-parroted refrain that the key to losing or maintaining weight is as simple as a "calories ingested = calories burned" equation, citing other factors like positive hormone control, the relative influence of certain types of exercise and the relative influence of the types of calories ingested. It now appears that, in addition to the aforementioned factors consigning the "it's only calories in = calories out" maxim to the remainder bin of overly-simplistic scientific wive's tales, we've got a new dimension: human adenoviruses:

A growing body of obesity research focusing on human adenoviruses and brain hormones may prove that being fat is not your fault.

Researchers at the University of Wisconsin, Madison, found that chickens infected with human adenovirus 37, or AD-36, packed on up to three times more fat than normal chickens. "This is a novel concept as far as a cause for obesity.

If these viruses are contributing to obesity, as we get to the point of developing a vaccine, there may be a way to help combat the obesity epidemic," said Leah Whigham, the study's lead author and an associate scientist at the university.

Another human adenovirus, called AD-36, has already been linked to obesity in animals and humans. Whigham inoculated chickens with AD-2, AD-31 and AD-37, three of about 50 human adenoviruses known to exist. She made sure the animals' blood contained antibodies, which means the virus had infected them.

After four weeks both the inoculated chickens and a control group were analyzed for their weight and fat content. AD-2 and AD-31 showed no effect on the animals, yet AD-37 dramatically fattened the animals. The virus works directly on fat cells, increasing their adiposity, or fat content.

Neither the inoculated group nor the control group differed in their food intake, ruling out the possibility of overeating as a cause.

Wow, some cases of obesity may have a viral component. (Put down the twinkie, chico, we're talking "some.")

Also interesting is the research focus on hormones and brain chemicals:

In a second study, researchers in England identified a brain hormone, or a peptide, not previously known to cause obesity in children. The hormone, beta-MSH, is produced by a protein called proopiomelanocortin, or POMC, which is located in the hypothalamus of the brain. The beta-MSH hormone, along with alpha-MSH, control appetite and satiation, or the feeling of being full. When this hormone mutates, it can not work properly to monitor appetite.

But the article makes a direct error here:

Researchers know another POMC peptide, called alpha-MSH, causes obesity, but beta-MSH was the "neglected sibling" -- largely because it is not expressed in rodents, said Stephen O'Rahilly, a professor of medicine at the Cambridge Institute for Medical Research in England.

Alpha-MSH (alpha-melanocyte stimulating hormone), is a substance produced in the pituitary gland that does a bunch of interesting things, primarily famous for causing you to tan ("melanocyte stimulating"). It also has the effects of increasing libido, reducing inflammation and serving as an appetite supressant, maybe working on an alternating axis with the aforementioned beta-MSH to regulate appetite (perhaps "on-off" switches; I'm guessing on the interaction, as I don't know beta-MSH well). So alpha-MSH does not "cause obesity" as the article states, rather the opposite: a lack of the substance (or gene that makes it) causes obesity.

Interesting to note: two drugs are currently in development that synthetically mimic and augment the action of alpha-MSH; one focusing on tanning by drug, another focusing on enhancing libido equally well in men and women (Full disclosure: I own stock in both companies). Each of the drugs has been developed with the goal of isolating the primary actions (tanning and libido), but appetite suppressant side effects remain, and a melanocortin-based anti-obesity treatment may be closer than you think.

You know what would cure the Obesity epidemic here in the US? A good famine. People can tweak this stuff all they want, but for the MOST PART- it is calories in, versus physical output. I say this as a woman who has lost almost 20 lbs in the last month because I haven't been eating enough. When I can eat? Carbs carbs carbs.

Followed your link to the Atkins article and you explained a problem that did crop up for me. Confusion was a problem for me when I forgot to eat enough carbs...sort of a vicious circle, get stupid because you aren't eating enough carbs, stay stupid because you aren't eating enough carbs...forget to eat carbs. hehe.

That's a prime factor. But like "most" people, you feel confident about a broad proclamation based on your experience, as a single human living on a planet of billions.

Some (many, most) people can lose weight by simply eating less. Among the majority of those people, not only is a primarily-carbohydrate diet ill-advised because it's nutritionally bankrupt, it will also comparatively cause them to retain more fat and lose muscle. But other people have damaged @-MSH genes, adenoviruses or malfunctioning thyroid glands - among other unknown factors - that make losing weight immeasurably harder. Also underplayed is the idea of "metabolic set-points," namely how getting fat once synergistically encourages one to stay fat, via a variety of self-reinforcing metabolic processes. Another example: what type of exercise is a better strategy to lose fat: aerobic or anerobic weight-bearing? One is much more strategically effective than the other, based on how it impacts your hormonal chemistry and resting metabolism.

The overall point being, it's far more complex than just "stop eating so much" with many people. At the very least, dieticians and physicians should know this.

To wit:

In the United States obesity rates have doubled for adults and tripled for children in the last 30 years. Atkinson pointed out the epidemic is global: Several developing countries, such as Paraguay and South Africa, have comparable rates of obesity as Americans.

"It becomes hard to say we're eating more McDonalds. Now we're asking, 'What's going on here?'" he said.

I guess the problem I have with some of the examples you give - and with such weight loss advice - is that for the majority of people, it isn't a metabolic issue. They aren't "carb addicts" or whatever trendy that's-why-I'm-overweight excuse they're using this week.

Junk food- don't eat it.
Meals- smaller portions.
general diet- balanced.

I know people who have been on the Adkins diet for years. They lost weight initially, true, but it didn't lead to them becoming a "fit" person. Yet, holiday after holiday I have to endure the "I can't eat that" mantra. Perhaps it's made me bitter.

I don't quite agree with that statement - because once an individual initially failed to exercise self-control at a certain age, it became a "metabolic issue" for a lot of them, because fat metabolically doesn't want to leave once it's created. And metabolic efficiency declines with age.

That being said, I think that we can both agree that self-control is still the main factor in health, exercise and weight-loss. There are just better/more efficient ways to apply oneself to getting fit besides thinking solely in terms of "calories in less than calories out" is one of my points, as well as the fact that you can't look at someone that's overweight and positively assess that they're simply lazy.

They aren't "carb addicts" or whatever trendy that's-why-I'm-overweight excuse they're using this week.

I get annoyed by "trendy that's-why-I'm-overweight excuses)" myself, and trendy "this is what diet I'm on this week" excuses as well.

But mostly that's a symptom of humans being perpetually-annoying, whiney creatures, I think. Just my pet theory.

We can find a gene associated with everything...homesexuality, cleptomania, obesity, violent tendencies...whatever, but in the end, people still need to be responsible for their actions!

First of all, your body stores energy in more forms than just fat, and this comment reveals your level of education on metabolism.

Second, I'm sorry if this is "too much for you" to grasp, John, but in what form you take in the energy, what machinery your individual body has available to process and expend vs. store the energy and what methods you choose to expend the energy are all key and highly variable variables in the equation, contradicting the simple "calories in vs. calories out" equation.

I could eat 300 calories of protein vs. 300 calories of sugar, and my body will utilize the material to drastically different effect. And if my body has a defective gene responsible for utilizing blood sugar or controlling appetite, my machinery has certain inefficiences less able to be influenced by your simplistic equation.

That's because the individuals were military men that underwent STARVATION, which is neither here nor there in an intelligent discussion about the rational influences on body composition.

In some instances, obesity can be the product of systemic inflammation. Chronic cryptopathic systemic inflammation is at the root of many midife diseases: cvd, adult onset diabetes, arthritis, etc. Have your doctor keep a close watch on your CRP (c-reactive protein) and do what you must to keep those levels within normal range.
